The first cancer-directed therapy (treatment given in an attempt to destroy or modify cancer tissue) was surgery (excisional biopsy and cervical lymph node excision). The shave biopsy incompletely excised the lesion according to the surgical observations. Do not code as an excisional biopsy when there is macroscopic residual disease.

Shave biopsy followed by a gross excision of the lesion. Margins are less than 1 cm. If the excision does not have microscopically negative margins greater than 1 cm, use the appropriate code 20-36.

Code the most extensive surgery if the patient has multiple surgical procedures of the primary site.
