72 year old male was seen with elevated PSA. Rectal: Prostate gland moderately enlarged with each lobe about 2.5 x 1.5 cm; relatively flat and smooth.

CT scan: moderately enlarged prostate without distinct extracapsular extension or asymmetry of seminal vesicles.
09/15/XX Chest X-ray normal. Bone scan: no metastatic disease.
   
Scopes 11/05/XX Cystoscopy: urethra normal, bladder mildly trabeculated.
   
Laboratory

PSA elevated 10.5 ng/ml
   
Pathological Reports

09/03/XX

Prostate needle biopsy: L prostate Gleason 6 adenocarcinoma. R prostate Gleason 7 (4+3) adenocarcinoma.

   
Treatment

Radiation: Implantation of 102 I-125 seeds for a total of 25.7mCi
